How is P4L Medical Fitness and Wellness Center different than a gym or health club?
The biggest difference between Perform4Life and other fitness facilities is that we focus on overall wellbeing. We help members through a variety of processes to reach their goals. We focus on the whole person to help our clients lose weight, gain muscle, improve strength, and to encourage a healthy lifestyle. Our experts are prepared to guide our members safely through the process. Process is always progress over perfection.
Where is P4L located? Do you have an elevator?
We are located at 841 W. Washington Street, on the second floor of the Great Lakes Sports Medicine Institute. Please enter on the east side of the building where the elevator is located.
What are your business office hours?
Monday – Thursday | 8 AM – 6 PM
Friday | 8 AM – 5 PM
Saturday | 9 AM – 12 PM
Do you have WI-FI?
Do you have lockers and showers?
Do you provide hand towels and bath towels?
We provide hand towels. Please bring your own bath towel.

Are group exercise classes available?
Perform4Life currently offers small group training for our members. Small groups are created for 2 – 6 individuals. Class times verify. Please sign up through the mywellness app, on the unity kiosk, or by contacting member services.
What is a lifestyle Assessment?
Your journey begins with a comprehensive lifestyle assessment to define your starting point, identify physical limitations and establish a baseline to measure progress. Information collected during the assessment is used by the exercise specialist to develop a personalized exercise prescription plan for each member. After we review the results of your assessment with you, our exercise specialists will work with you to select the most appropriate exercise prescription for you. Prescriptions are not required but highly recommended. Everything moving forward is built around you, championed by our team of experts who explain, inspire, and affirm.
MEMBERSHIPS AND OTHER RULES
What is the guest policy?
If you are interested in bringing a guest to use the facility, please call during staffed hours to purchase a day pass. Guest passes are only to be used during regular business hours unless prior arrangements have been made with management. For your own security, members and guests with day passes are the only individuals allowed to use the facility.
Can I add my spouse to my membership?
Yes, please call member benefits at 906-226-7456.
Can I try before joining?
We offer 7 consecutive day VIP passes for those interested in joining our Wellness Center. These are not intended for people that live out of town. You must have valid identification and call to arrange an appointment during our business hours.
What should I wear to exercise?
Comfortable exercise apparel and proper gym/tennis shoes. No street shoes in the fitness center.
Can I freeze my membership?
Freeze Policy: You can freeze your membership once in a calendar year for the following reasons: medical, moving, and military. You must come into the office with the proper proof prior to freezing your membership. Please contact member services at 906-226-7456.
Can I cancel my membership?
Cancellation Policy: You can cancel your membership for the following reasons: moving, medical and military. Members must provide a 30-day notice and provide proof prior. You will be responsible for any dues during the 30-day period. Please contact our member services office to arrange an appointment.
Is there a student membership?
Yes, we offer NMU student memberships based on semesters.
Do I need to declare my health?
Everybody who is a member must go through a Physical Activity Readiness Questionnaire (PAR-Q). If you have a chronic health condition, please check with your healthcare provider prior to engaging in an exercise program. For your safety and well-being, please bring a physician release with you.

I lost my key; how do I replace it?
Stop in the member services office to take care of this at a cost of $40.00.
Can I buy gift cards for friends or families that are not members?
We do not offer gift cards at this time. We do offer 7-day VIP passes for them to try during regular business hours.
How old do you need to be to become a member?
You must be at least 14 years old in order to become a member. Children ages 9-13 are not permitted in the facility unless they are training one-on-one with an exercise specialist.
